What Are They?
Regular check-ins that connect every part of your child’s care
A child’s care rarely happens in just one place. Between therapy sessions, school, and outside specialists, it is easy for goals to get out of sync or for progress in one setting to go unnoticed in another. Coordinated Care Conferences bring the people involved in your child’s care together on a regular basis to share updates, align on goals, and solve problems before they grow.
These meetings are a required part of the EIDBI benefit, and at Guidance Center we treat them as a genuine opportunity to make sure your child’s plan is working everywhere, not just a box to check.

How often do Coordinated Care Conferences happen?
Frequency depends on your child’s plan, but conferences typically happen at set intervals as part of ongoing EIDBI care, with additional meetings scheduled if a specific issue comes up.
Do I have to attend every conference?
Family participation is a core part of these conferences, and we work to schedule them at times that work for your availability.
Will my child’s school be involved?
When it is helpful, yes. We can coordinate with school staff or an IEP team to make sure therapy goals and school supports are aligned.
What if we disagree with something discussed in the conference?
These conferences are meant to be collaborative. Your input as a parent or caregiver directly shapes decisions about your child’s care.
Are Coordinated Care Conferences covered by insurance?
Yes. As part of the EIDBI benefit, care coordination is covered for most families enrolled in Medical Assistance, MinnesotaCare, or MA-TEFRA.
